Patient Enrolment

For patients that are eligible:

Step 1: Explain the program to the patient (please see guidelines for assistance).

Step 2: If the patient agrees to join the program, complete survey as part of the enrolment process.

Within the survey you have the following forms:

  1. Patient Consent form
  2. EQ-5D-5L which is a patient outcome measure- understanding how patients are feeling based on 5 key areas of wellbeing
  3. Frailty Screening Questions

Once you have completed these surveys with the patient, please complete the link below.
