Information for Prospective KWIP patients

Consent – Patient or Carer

Overview

Your GP has enrolled you in the “Keeping Well and Independent Program”, a joint program between GPs in Northern Sydney, the Sydney North Health Network (SNHN) and the Northern Sydney Local Health District (NSLHD).

Benefits to you

As part of this program you will have access to the following:

  • a Geriatrician from the hospital that can help by providing input into your care which will be facilitated through your GP
  • support from a KWIP Coordinator which can help you get access to health and social support services
  • information on after hours GPs
  • the ‘Rapid Outreach Service’ which works alongside ambulance in reaching out to patients at a time of urgent need or crisis.

In addition to all these services your GP will schedule in regular appointments with you to ensure that you have regular contact with the GP and that your care plan is helping you achieve your health goals.

Information

As part of the program, your health information will be shared securely with appropriate health and social service clinicians to better support your care. Your information will be treated as highly confidential and will not be used for any other purposes outside of the program.

Evaluation

Within the survey, you will complete the EQ-5D-5L questionnaire and the Frailty Screening Assessment tool known as ‘the FRAIL Scale’ will help the Program with the evaluation process. These surveys help clinicians understand how to further assist with your health and social care needs. These surveys will be completed as part of the patient enrolment process and asked again at the 12 month enrolment date.
