Expression of Interest Black Dog Institute StepCare Service

StepCare is an innovative digital mental health assessment and intervention program delivered in general practice waiting rooms. StepCare supports GPs to provide timely, cost-efficient early intervention and treatment for patients 18 and over. Sydney North Primary Health Network is currently seeking Expression of Interests from general practices willing to implement the StepCare Service within their practice for 12 months commencing November 2018.

Frailty Screening and Management Initiative

HealthDirect Australia, the Departments of Health, the Department of Social Services and Strata Health are currently developing a Proof of Concept (PoC) to support primary care clinicians in completing the e-assessment application form (by integrating the primary care clinical management systems with the government My Aged Care systems).

EOI: Integrated Partnership Model of Care for COPD

Sydney North Primary Health Network is partnering with the Chronic Disease Rehabilitation Service (CDCRS) at Northern Sydney Local Health District, the University of Sydney and the University of Wollongong to trial a new model of care for people with COPD.
