How do we help our Community?

Internationally, the ageing population and increasing prevalence of chronic disease, combined with improvements in technologies and treatment, has resulted in significant changes in the way healthcare is delivered.

As the focus shifts towards provision of more care in the community, outside of hospital settings, the role of Primary Health Networks in driving high quality, integrated care becomes increasingly important.

Primary healthcare is the first point of contact for most people within the health system. This can occur through general practice, allied health services, community health organisations, or local pharmacies to help ensure the most efficient use of the health system to achieve the best patient outcomes.

The Primary Health Network name reflects the critical role that PHNs play in networking health services across local communities so that patients, particularly those needing coordinated care, have the best access to a range of healthcare providers.

SNHN has implemented a range of new initiatives and introduced new tools and programs to build capacity in primary healthcare to manage the increasing complexity that comes with providing care closer to home. As a Primary Health Network, a strong focus is also placed on developing strategic partnerships to support hospital avoidance. This website highlights SNHNs successes to date through the commissioning of services in response to the unique health needs of the Sydney North community. Over the next five years (2018-2023) we will continue to work closely with our providers to maximise the impact of these services and expand our role to build and improve the health literacy of our community.
