How do we help our Community?

Internationally, the ageing population and increasing prevalence of chronic disease, combined with improvements in technologies and treatment, has resulted in significant changes in the way healthcare is delivered.

Primary healthcare is the first point of contact for most people within the health system. This can occur through general practice, allied health services, community health organisations, or local pharmacies to help ensure the most efficient use of the health system to achieve the best patient outcomes.

The Primary Health Network name reflects the critical role that PHNs play in networking health services across local communities so that patients, particularly those needing coordinated care, have the best access to a range of healthcare providers.

At Sydney North Health Network (SNHN), we will take a ‘place-based’ (local) approach to catalyse change and achieve our vision. Our approach recognises that health across the lifespan is influenced by numerous factors within that place, including physical, social, and economic elements. This holistic approach ensures that health improvements are not isolated but interconnected with other aspects of wellbeing. This approach will empower communities to actively participate in decision making and shape their own “best health” outcomes.

To inform our future strategic direction, we carried out a population needs assessment for Northern Sydney to form a baseline understanding of areas of health inequity. Overall, more than 100 individuals attended these community engagement consultations, including commissioned providers. To view our 2024-2029 Strategic Plan click here
