What is HealthPathways?

HealthPathways is a free, peer-reviewed local health information website, designed to support GPs, practice nurses, and hospital and primary care clinicians in the community with:

  • service navigation
  • condition management
  • referral to other services
  • reference materials and patient resources.

The aim is to improve patient access to the right care, in the right place, at the right time, and to better integrate primary and specialist systems of care. Sydney North HealthPathways is a collaboration between the Sydney North Health Network (SNHN) and the Northern Sydney Local Health District (NSLHD).

HealthPathways is not designed to be used by the public or by patients.

Why is HealthPathways useful right now?

HealthPathways is useful when needing referral information on various local services to assist you to manage your patients.

It is used in this Program as a tool to provide you with options on the various services you can refer to.

There are a few various ‘pathways’ that this Program may refer you to review. Below are some examples:

How to Sign up

Please send an Email to the below account requesting that you are a GP, and you would like to gain a log in and password for health pathways.
