  • Describe the concept of the Primary Health Network, HammondCare End of Life Project in Residential Aged Care.
  • Learn how to implement resources developed for the project in your practice: Flip chart, End of Life Resource Booklet, Lanyard cards
  • Implement contemporary evidenced based end of life care in residential aged care
  • Identify directions regarding end of life care and Dementia
