Sydney North Health Network (SNHN) is offering general practices in the region the opportunity to complete a Quality Improvement activity aligned with improving immunisation rates. SNHN will support practices by providing an incentive to complete a Quality Improvement activity on improving winter wellness focusing on delivery of Influenza, Covid-19 and Pneumococcal vaccinations to eligible populations. Practices can use either one or a combination of the following tools to plan immunisation activities:
- Primary Sense reports
- Clinical Management software reports
- AIR reports
SNHN will incentivize practices $200.00 per activity, up to a maximum of $400.00 excl. GST (i.e., 2 x activities).
To take part in this activity, please do the following:
- Complete the Expression of Interest Form by Wednesday 3rd April 2024
- Identify appropriate eligible populations using the above-mentioned tools and plan your Quality Improvement Activity using the attached templates and examples as a guide.
- Submit your completed Quality Improvement Activity via email to pcait@snhn.org.au no later than 10th June 2024.
The Primary Care Team will be in contact with your nominated primary contact after receipt of your completed activity.
