Expression of Interest: LCN Steering Committee Member

As part of our engagement with local communities, SNPHN has established six Local Coordinated Networks (LCNs) across the Northern Sydney region. These are groups of GPs, primary healthcare practitioners and local service providers based around specific geographic areas

LCNs aim to:

  • Enhance understanding of your local community’s health needs and priorities,
  • Facilitate the provision of care based on local need,
  • Improve data usage to better measure the impact of health services, and
  • Ultimately improve health outcomes in your region.

Each LCN will be locally driven and we are currently looking for health care providers who are interested in being part of their local Steering Committee.  To apply to sit on your LCN committee submit your details below.
