Sydney North Health Network (SNHN) is seeking expressions of interest from GPs who currently visit aged care facilities.
We are currently working with a selection of aged care facilities in Northern Sydney to support them to connect to and use My health Record regularly as part of their daily practice. Part of the process involves working with GPs to upload event summaries into My Health Record either from the facility software or their own practice software. The benefits of using My Health Record in aged care facilities are:
- Faster access to discharge summaries
- Access to residents’ previous medical history
- Reduction in time spent entering clinical notes into various systems
- Access to prescription and dispense records
