Sydney North Health Network (SNHN) would like your input into designing an approach to fund the set up and improvement of virtual health capabilities for residential aged care facilities (RACFs) in the Northern Sydney PHN region, so that residents can readily access virtual care with primary healthcare professionals and other clinical settings when needed.

The approach is intended to augment and align with existing RACF telehealth programs and capabilities. All telehealth modalities are in scope. Also in scope is improving digital health systems or enablers within RACFs that help to increase access and quality of telehealth consults.

We are looking to co-design an approach that is best suited for the Northern Sydney PHN region with healthcare providers. We would love to hear from people with lived experience and subject matter experts, like yourselves.

The main objective is to get attendees to provide their thoughts on the successes, challenges and potential areas for improvement for telehealth and digital system enablers within RACFs.
