Northern Sydney Nurses Network Meeting: Care Planning for Medical Director

Sydney North Network invites primary health nurses to this networking and education event.

The meeting will provide a networking opportunity as well as an update from Ms. Julia Spark, Practice Manager East Ryde Family Medical Centre & Nurse Kerry McBride, Nurse Co-ordinator SNHN.

Discussion will include: Medicare requirements for GP Management Plans (GPMPs) & Team Care Arrangements (TCAs), Chronic disease GPMP templates, importing and editing word templates into MD, how to use Auto fills to your best advantage, medical goals for care planning, accessing patient information in My Health Record, Proda, AIR, NCSR, using MD software to complete GPMP and TCA, setting up a care Plan template on MD and a brief introduction to Smart Goals and motivational interviewing.
