NDIS: Participant/Patient and Carer Journey

This webinar is being held in collaboration with Your Side Australia and will cover NDIS eligibility requirements, what GP’s can do to assist in the application process, patient and carer resources, referral pathways, and YourSide NDIS and Carer Gateway services.

YourSide support people over the age of 18, living in the Greater Sydney region who have a range of disabilities.  They assist with a number of services including personal care, community access, allied health and other activities of daily living.

Brittany Jack – Director, Business Growth – Your Side Australia
Brittany’s experience in human services includes youth, employment, education, homelessness, justice, and now disability and aged care. She has significant experience in UK and Australian contexts delivering large scale, complex funded programs (including NDIS strategy and change management) in urban and regional environments.

Brittany is passionate about ensuring the social impact is realised in a way that ensures all of us can live full, rewarding lives. She seeks best practice and innovation and is motivated by seeing individuals and communities thrive.

GPs, Practice Nurses & Allied Health Professionals
LEARNING OUTCOMES:
  • Describe NDIS eligibility requirements
  • Identify support available for patients and carers with applying for the NDIS
  • Describe referral pathways and support available to patients and their carers
  • Assess overlap between NDIS Participants and their carers and how GPs can support them on their journey
