Managing Diabetes and Mental Health

General Practitioners, Practice Nurses, Credentialled Diabetes Educators and Allied Health Professionals are invited to a workshop on Diabetes and Mental Health.

Topics will include:

  • Managing patients with diabetes and mental illness, dementia or cognitive decline, and anxiety & depression
  • There will be an opportunity for rigorous Q & A and discussion with a panel of multi-disciplinary healthcare professionals
  • Streamlining digital communication in the management of diabetes
  • Community services and resources to improve connection & health literacy for the isolated,  CALD populations and the less-abled.

  • Access multi-disciplinary healthcare professionals to provide comprehensive and holistic care
  • Access  eReferrals/discharge health summaries (MHR) and other digital interfaces to enhance timely communication between health care professionals
  • Collaborate with local Community Services to improve connection, access and health literacy for the isolated, CALD populations and the less-abled.
COST:
There is no cost for GPs, PNs, Diabetes Credentialled Educators & AHP working within the SNHN region to attend this training
