Face to Face: Primary Care Reforms and Quality Improvement General Practice

GPs and Practice Care Teams are invited to attend this education session providing the latest information on the Strengthening Medicare and the budget and how you can embrace the changes to improve your current practice and patient outcomes. We will explore how to use data and team to inform quality improvement and improve patient management and care.

The education session will be facilitated by Dr Walid Jammal, principal GP with a keen interest in health economics and models of care in general practice, especially the Patient Centred Medical Home framework; and Dr Kirsten Meisinger, MD who is an international expert on Patient Centred Medical Homes and healthcare system transformation.

Participants will learn and explore the principles of team-based care and their application in General Practice as well as aligning this to the use of quality improvement tools and techniques.

Lunch will be provided between 12-12.45pm

SPEAKERS:

Dr Kirsten Meisinger | MD

Dr Kirsten Meisinger, MD is an international expert on Patient Centred Medical Homes and healthcare system transformation. Dr Meisinger is faculty lead for the System Transformation Team at the Harvard Center. Kirsten is a MD and cares for an active Family Medicine panel, an award-winning Patient Centred Medical Home.

Dr Walid Jammal, GP

Dr Jammal is a principal GP with a keen interest in health economics and models of care in general practice, especially the Patient Centred Medical Home framework.

TARGET AUDIENCE:

GPs and Practice Staff working in the Sydney North Health Network region

LEARNING OUTCOMES:

• Utilise practice data to perform the essential elements of quality improvement in General Practice
• Determine what MyMedicare and VPR could mean to you and your patients
• Interpret the current GP reform environment
• Differentiate what data driven team-based care could look like

COST:
There is no cost for Practices within the SNHN region to attend this training
