Face to Face: Geriatric Evaluation, Management and Frailty

Sydney North Health Network (SNHN) invites local GPs, Primary Care Nurses, Allied Health Professionals, Pharmacists, RACF and their relevant staff to this education session providing:

– An update on the Geriatric Evaluation and Management (GEM) unit and frailty with Dr Gillian Meyer
– Information on the Beaches Rapid Access Care of the Elderly (BRACE) Model of Care with Sheridan Mitternacht
– An overview of HITH (Hospital In The Home, previously APAC) including referral for clinical conditions such as cellulitis, wound infections, pneumonia, urinary tract infections, pyelonephritis, COPD, heart failure and thromboembolic disease requiring Hospital In The Home treatment.

SPEAKERS:

Dr Gillian Meyer | Geriatrician, GEM (Geriatric Evaluation and Management) Unit, NSLHD

Dr Gillian Meyer is the clinical lead for the GEM unit at Mona Vale Hospital. She has a specialty interest in looking after frail elderly patients, and enjoys working within a multi-disciplinary environment with the common goal of returning patients to living well in the community. She also works as a VMO at the Northern Beaches Hospital, and has clinics in Frenchs Forest. She enjoys pastries and sunshine.

Sheridan Mitternacht | Clinical Nurse Consultant, BRACE team (Beaches Rapid Access care of the Elderly), NSLHD

Jairo Herrera | HITH (Hospital in the Home), NSLHD

Jairo is a registered nurse by background and has worked in Hospital in the Home for over 15 years. Jairo has a strong history of collaborating with GPs and together with the former Divisions of General Practices was recognised by the NSW Health Awards for strengthening Primary Health and continuing care in the community.

TARGET AUDIENCE:

General Practitioners working in the Sydney North Health Network region. The education will be delivered towards supporting GPs with patient referrals, however other health profssionals and support staff may find it of interest.

LEARNING OUTCOMES:
  • Recognise the condition of frailty and it’s complications
  • Identify potential patients that are suitable for Hospital in the Home service
  • Differentiate referrals to BRACE with regard to what is ‘in scope’ and ‘out of scope’
  • Avoid patient hospitalisation through safe and intensive referral to HITH for management of various common clinical conditions
