Face to Face: Enhanced Primary Care: Working together to provide preventative and person centred, multidisciplinary care

Join us for an evening of networking, dinner & keynote presentation by our international presenter Dr Kirstin Meisinger, MD

Sydney North Health Network (SNHN) invites GPs, Specialists, Practice Nurses, Pharmacists, Aged Care Providers and Allied Health Professionals to the final event in our Person-Centred, Multidisciplinary Care Symposium Series. This third session will focus on Enhanced Primary Care with world leading expert Dr Kirstin Meisinger and will include a panel of speakers discussing the importance of an integrated system and what it means to have a person-centred approach in primary care.

SPEAKERS:

Dr Kirsten Meisinger | MD

Dr Kirsten Meisinger, MD is an international expert on Patient Centred Medical Homes and healthcare system transformation. Dr Meisinger is faculty lead for the System Transformation Team at the Harvard Center. Kirsten is a MD and cares for an active Family Medicine panel, an award-winning Patient Centred Medical Home.

PANELLISTS:

Dr Magda Campbell, GP

Dr Magda Campbell is a GP and principle of her own Practice at Pennant Hills. Magda is a Fellow of the RACGP with more than 18 years’ experience in the Divisions of General Practice. Dr Campbell is Co-Chair of the SNHN Clinical Council and a member of the Board.

Dr Walid Jammal, GP

Dr Jammal is a principal GP with a keen interest in health economics and models of care in general practice, especially the Patient Centred Medical Home framework.

Errol Lim, Physiotherapist

Errol Lim has been a physiotherapist for 25 years. He is co-founder and managing director of BJC Health, a multidisciplinary rheumatology centre with 3 clinics in Sydney

Sue Strahan, Practice Nurse

TARGET AUDIENCE:
