Sydney North Health Network (SNHN) invites local invites local GPs, Nurses, Practice Managers and Allied Health Professionals to a workshop to identify strategies for optimally managing diabetes in general practice.

CHERYL UREN, Primary Care Coordinator, SNHN – Review practice management billing for diabetes care

JENNY HUGHES, Primary Care Coordinator, SNHN – Use Primary Sense for diabetes quality improvement (QI)

KARA MCDONNELL, Health Promotion Officer, NSLHD – Referral pathways for local services available in the community including Go4Fun and Get Healthy

SANDY WU, Integrated Care Clinical Nurse Consultant and CDE – Close to Home and Shared Diabetes Care in General Practice programs

TARGET AUDIENCE:

GPs, Nurses, Practice Managers and Allied Health Professionals

LEARNING OUTCOMES:
  • Optimise and streamline diabetes care in general practice
  • Recognise practice management billing for diabetes care
  • Demonstrate a good working knowledge of Primary Sense to facilitate quality improvement in practice
  • Foster a collaborative, multidisciplinary approach to diabetes in our community through an awareness of the Close to Home and Shared Diabetes Care in General Practice programs
  • Recognise, promote and appropriately refer to local available programs.
COST:
There is no cost for Practices and their staff within the SNHN region to attend this training
