Face to Face: Improving Outcomes for Patients with Dementia

We welcome all General Practice teams to attend this dinner meeting.

Dementia Australia will provide an overview of education, services and support available to people living with dementia, and there will be presentations on a range of quality improvement initiatives in general practice, including an Ageing Well Screening Tool. It will also be an opportunity to share and discuss dementia care and quality improvement strategies, successes and challenges, with other practices with an interest in dementia.

TARGET AUDIENCE:

GP’s, Practice Nurses, Practice Managers and other interested Health Professionals working in the Sydney North Health Network region.

LEARNING OUTCOMES:
  • Explore the range of education, services and support available through Dementia Australia
  • Share and discuss strategies to improve dementia identification and management in practice and risk reduction opportunities.
  • Review outcomes of quality improvement activities including successes and challenges and next step.
COST:
There is no cost for Practices and their staff within the SNHN region to attend this training
