FACE TO FACE – Case conferencing in Diabetes Care: Working together to provide person centered, multidisciplinary care

Join us for an evening of networking, dinner and education in the management and coordination of Diabetes care.

Sydney North Health Network (SNHN) invites GPs, Specialists, Practice Nurses, Pharmacists, Aged Care Providers and Allied Health Professionals to the second event in our Person-Centred, Multidisciplinary Care Symposium Series.

This second session will focus on the management of Diabetes and will include keynote speakers and a panel of experts for questions and discussion about accessing services and patient centred care. Presentations will include patient case conferencing for those patients who do not meet the referral criteria for the Diabetes Assessment Unit as well as a presentation on SGLT2 medications and perioperative care.
