About Sydney North Health Network

The Northern Sydney PHN – operated by the Sydney North Health Network (SNHN) – began operations on 1 July 2015. The Sydney North Health Network is one of 31 Primary Health Networks (PHNs) established by the Australian Government to increase the efficiency and effectiveness of medical services for the community. Our focus is on patients who are at risk of poor health outcomes and we work to improve the coordination of their care so they receive the right care, in the right place at the right time.

Better health outcomes for patients is achieved by working together with a network of health professionals including General Practitioners, Practice Nurses, Allied Health Providers, the Northern Sydney Local Health District and other health services. This partnership approach and community focus is reflected in our vision: Achieving togetherbetter health, better care.

Sydney North Health Network aims to simplify the healthcare system by providing better information, education and networking opportunities for Health Professionals, connections across the health system, a better experience for health providers and better health for the community.

Membership Information

Membership of Sydney North Health Network (SNHN) means being part of a local organisation at the forefront of connecting and coordinating health services for our local community. SNHN works directly with primary healthcare providers, secondary care providers, hospitals and communities to:

  1. Increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes.
  2. Improve coordination of care to ensure patients receive the right care, in the right place, at the right time.

Membership of SNPHN is open to ORGANISATIONS, INDIVIDUALS and ASSOCIATES with a recognised role in primary healthcare in the Northern Sydney Region.

SNHN Ltd is a company limited by guarantee which is governed by a Board of Directors. This means that it does not have shares or shareholders, instead it has members. By becoming a member you will have the opportunity to be involved in our organisation.

Benefits of becoming an ORGANISATION or INDIVIDUAL Member of Sydney North Health Network include:

  • Voting rights at Board elections;
  • Opportunity to have your say on current and future health programs;
  • Access to member only continuing professional education events;
  • An invitation to attend the Annual General Meeting;
  • 20% off the advertising rates on the SNHN eNewsletter;
  • 20% off mail-out rates with SNHN;
  • Copy of the Annual Report; and
  • eNewsletter monthly subscription.

Benefits of becoming an ASSOCIATE Member of Sydney North Health Network include:

  • Opportunity to have your say on current and future programs;
  • Priority access to continuing professional education events, support, services, resources, advice, and networking events administered by the SNHN;
  • An invitation to attend the Annual General Meeting;
  • 10% off the advertising rates on the SNHN eNewsletter;
  • 10% off mail-out rates with SNHN;
  • Copy of the Annual Report; and
  • eNewsletter monthly subscription.
  1. An ASSOCIATE that does not meet the individual or organisation membership criteria is eligible to become a member if they are involved or interested in primary healthcare in the Northern Sydney area and can demonstrate alignment to the strategic objectives of the company.
    For example:
  • Practice Administrator
  • Practice Reception

Note: An Associate Member can attend the AGM but doesn’t possess any voting rights.

  1. An ORGANISATION is eligible to become a member if the organisation is one of the following:
    • An Organisation which has a presence within the Northern Sydney area reasonably acceptable to the Board, can demonstrate alignment to the strategic objectives of the Company and meets one or more of the following requirements:
      • is a Peak Body;
      • is a Service Provider; or
      • is an Organisation that represents the community or consumers in the area, and has demonstrated leadership and capability in integrated healthcare delivery in the Northern Sydney area.
Membership Application Process
  1. Applicant to complete membership application form in this pack and send via post or submit online – http://snhn.org.au/about-us/membership/
  2. Company Secretary to check the nominated member details and confirm the applicant’s agreement to the membership conditions.
  3. Company Secretary to recommend applicant to the Board.
  4. Board to approve or reject membership, or request additional documentation from the applicant to demonstrate its application.
  5. Board does not have to give reasons for rejecting an application.
  6. Applicant to be advised of outcome by Company Secretary, and, if successful, entered on the Membership Register.

Subscription Fees

Membership for 2021 is free.

Membership of SNHN will continue to be offered to all health professionals throughout the Northern Sydney region at no cost during 2021.

To further enhance your membership experience, SNHN will soon be offering a range of new services for members which may attract a small fee. Details regarding these services will be announced over the coming year.’
