Expression of Interest: Train-the-Trainer Workshop on Advance Care Planning
SNHN are seeking Expressions of Interest from GPs and Practice Nurses to attend a Train-the-Trainer workshop on Advance Care Planning (ACP). Upon completion of the workshop, it is anticipated that participants will be able to facilitate small group meetings with the aim of enhancing ACP in General Practice.
About the ACP workshops:
The process surrounding dying deserves the same quality improvement considerations as other aspects of healthcare. ACP can improve the likelihood that a person’s wishes will be known and respected at end of life, improve patient and family satisfaction with care, reduce inappropriate treatments at the end of life and diminish the recognised anxiety, depression and post-traumatic stress symptoms in surviving relatives.
Workshop Title – Advance Care Planning: do you really know what your patient wants?
This practical interactive workshop will cover the following components:
- What is ACP, what are its benefits and how is it relevant?
- What are the ethics and legalities of ACP and how do you incorporate it into medical practice?
- When and how to discuss ACP with patients?
- What MBS item numbers can be used for ACP?
Learning Objectives:
By the end of this activity, participants should be able to:
- Describe the ethico-legal principles of advance care planning
- Explain the value of, and need for, advance care planning
- Develop a system for identifying patients for whom advance care planning discussions are indicated
- Exercise the skills required to conduct an advance care planning discussion
- Incorporate advance care planning in your practice
- Facilitate small group meetings within the network
