Improving local health and healthcare needs

How Sydney North Health Network will commission primary care services from July 2026 is undergoing a transformation.

The new SNHN 2024-2029 Strategic Plan outlined the case for change. To catalyse this change, SNHN is taking a place-based commissioning approach to delivering services and targeting health inequity for people most at risk of falling through the gap.

SNHN is currently adopting a collective impact approach through the establishment of the Northern Sydney Wellbeing Collaborative. This collaborative is being made up of self-nominated representatives from across the health and social care sectors and people with a lived or living experience, advocates, carers and patients.

The Collaboratives will assist in the commissioning of hub-like services in Hornsby, Ryde and the Northern Beaches,  that will deliver integrated health and social care services with the aim to support health equity and improve the lifespan and health span gap.

About Us

The Northern Sydney PHN, operated by the Sydney North Health Network, is one of 31 Primary Health Networks (PHNs) established by the Australian Government to increase the efficiency and effectiveness of medical services for the community. Our focus is on patients who are at risk of poor health outcomes and we work to improve the coordination of their care so they receive the right care, in the right place at the right time.

Better health outcomes for patients is achieved by working together with a network of health professionals including general practitioners, practice nurses, allied health providers, the Northern Sydney Local Health District and other health services. This partnership approach and community focus is reflected in our vision:
