Introducing Neighbourhood Health Hubs

When health and wellbeing services work better together, people get better support.
Neighbourhood Health Hubs are being established to help create a more connected health and wellbeing system across Northern Sydney, making it easier for people to access the support they need, closer to home. These Hubs are local, place-based centres designed to improve how primary care, mental health, community services and wellbeing supports work together.
Developed alongside local communities, clinicians, service providers and people with lived experience, the hubs will improve connections between services and support more coordinated, person-centred care.
Neighbourhood Health Hubs are currently being developed across:
- Northern Beaches
- Hornsby
- Ryde
Each hub will be shaped by the unique needs and priorities of its local community.
Designed with communities, not just for them
Neighbourhood Health Hubs are being co-designed with community members, carers, health professionals, service providers and people with lived experience.
Through ongoing engagement and collaboration, local voices will help shape priorities, services and models of care, ensuring each hub reflects the needs of the people it serves.
Why Neighbourhood Health Hubs matter
The hubs are designed to:
- Improve coordination between health, wellbeing and community services
- Create clearer pathways to care and support
- Reduce duplication and fragmentation
- Strengthen partnerships between services and providers

By improving collaboration between services, the hubs will create a more connected and seamless experience for people seeking support.
Building the Hubs together
The hubs are currently in the design and development phase.
The first stage will bring together virtual multidisciplinary teams and partner organisations to trial new ways of working together. Early pilots will focus on priority areas including mental health, alcohol and other drug services, care coordination and other locally identified needs.
Over time, successful approaches may evolve into physical Neighbourhood Health Hubs, bringing services together to provide more integrated, wrap-around support for local communities.
As the hubs grow, they will continue to be shaped by community feedback, evidence and co-design.

The Wellbeing Collaboratives are the primary forums for engagement and co-design of the Neighbourhood Health Hubs.
