Greater Choice for At Home Palliative Care Lead
Make a difference in end-of-life care across Northern Sydney.
We are seeking an experienced and passionate Palliative Care Lead to lead the Greater Choice for at Home Palliative Care program and initiatives to increase awareness of; and facilitate and coordinate access to safe quality palliative and end of life care at home.
In this key role, you will lead the implementation of palliative care programs, work closely with healthcare providers and aged care services, build workforce capability through education and training, and strengthen partnerships across the community, primary care, and aged care sectors through awareness driving initiatives.
About You
You will have:
- Experience in health; aged care; palliative care; or primary care sectors
- Strong project management and stakeholder engagement skills
- The ability to lead service improvement initiatives and manage competing priorities
- Excellent communication, facilitation, and relationship-building skills
What You’ll Do
- Lead the Greater Choice for At Home Palliative Care program in the Northern Sydney region.
- Align delivery of the Greater Choice for At Home Palliative Care program with SNHN’s new Neighbourhood Health Hub model in Hornsby, Ryde and the Northern Beaches
- Engage with hospitals, general practice, residential aged care homes, councils community providers and people with palliative care needs and their families
- Coordinate education, workforce and community awareness activities
- Support improved care pathways and after-hours access to care
- Monitor outcomes and drive continuous quality improvement
This role presents a brilliant opportunity to bring your health or social care experience to contribute to meaningful system-wide change and improve the experience of people, families, and carers receiving palliative and end-of-life care.
Essential Criteria for the role:
- Bachelor of Health Sciences (or related) and post graduate qualifications or commensurate experience.
- Significant experience in health service planning, health project delivery or health policy review and improvement.
- Strong written and oral presentation skills.
- Strong inter-personal skills, with the ability to participate in advisory bodies and influence senior stakeholders in strategic operations.
Desirable Criteria for the role:
- Specialist knowledge and experience of palliative care, care coordination or service integration
- Experience of developing and delivering education and training
- Project management skills
- Experience of commissioning services
