Closure of Geriatrician Outreach to GP service from 30 June 2026

The Geriatrician Outreach to GP service was established in 2022 through Ministry of Health Collaborative Commissioning program funding to provide timely geriatrician support to GPs caring for older people in the community. The service was delivered by the Northern Sydney Local Health District and Sydney North Health Network.

From 1 July 2026, the Geriatrician Outreach to GP service will discontinue with completion of program funding. We understand this is disappointing news for Northern Sydney GPs who have engaged with the service.

Alternative geriatric services are available within Northern Sydney Local Health District to continue care for older patients from 1 July 2026. Please see here for these services and further information.
