Macquarie University Health is offering an integrated allied health service, funded by Sydney North Health Network, to support solo and small general practices across Ryde and Hornsby. This free service provides access to coordinated, multidisciplinary care for patients with chronic and complex conditions. With flexible face-to-face, telehealth or hybrid options, patients receive tailored, patient-centred support. A dedicated Care Navigator assists with care planning, case conferences, referrals and triage to make the process seamless for practices.\
