Primary Sense reports update, Jan 2026

A new Hepatitis C report is now available that can be utilised to identify patients with Hepatitis C, those at risk of Hepatitis C or re-infection, and to monitor treatments and tests conducted. 

This report allows practices to identify ‘Patients With Coded Diagnosis Of Hepatitis C’, and ‘Patients With Hepatitis C Risk Factors’. It lists information such as Diagnosis of Hepatitis C and the date first recorded, patients with recorded risk factors for Hepatitis C, Hep C medications, and the AST to Platelet Ratio Index (APRI) score.

The Chronic Kidney Disease (CKD) report has been updated to include two new columns: CKD Diagnosis and CKD Date.  

Practices will now have full visibility of CKD-related diagnoses and their corresponding dates across all relevant tables in the report. These fields are critical for understanding the clinical context and progression of CKD in patients, especially when determining eligibility for care plans or monitoring disease stages. Below are the benefits of this update: 

• Complete Clinical Picture: CKD diagnosis and date fields now appear consistently across all tables, supporting better clinical decision-making. 
• Improved Data Transparency: Practices can now verify when a CKD diagnosis was recorded, which is essential for care planning and compliance. 
• Supports Risk Stratification: Enables more accurate patient segmentation for Yellow, Orange, and Red Clinical Action Plans.
