New Integrated Team Care provider for Northern Sydney region

Integrated Team Care aims to improve health outcomes through the coordination of care and access to medical services, helping individuals better manage their chronic conditions. Grand Pacific Health (GPH) will now be managing the Integrated Team Care (ITC) Program for Aboriginal and Torres Strait Islander clients living with chronic conditions in Northern Sydney.

From 1 November 2025, referrals can be directed to the GPH’s ITC team:
