SPEAKERS:

Dr Kean-Seng Lim – Kean-Seng is a GP principle in a small group practice in Mt Druitt and a GP Supervisor. He has previously served on the RACGP Faculty Board and WSDGP Board. He has previously been a NEHTA Clinical Lead, as well as serving on the NPS eHealth Clinical Advisory Group and is currently the Vice President of the AMA (NSW) and on the Board of Wentwest – Western Sydney PHN.

TARGET AUDIENCE:

GPs, Primary Care Nurses, Pharmacists and Allied Health Professionals.

LEARNING OUTCOMES:
  • Introduce and discuss the person centred medical home model to achieve the quadruple aim – improved patient experience of care quality and population health, provider wellbeing and sustainable cost.
  • Discuss and explore the evidence of benefits to the practice, the person and the community.
  • Explore the requirements of PCMH implementation and practice readiness.
COST:
There is no cost for GPs and health professionals working within the Sydney North Health Network region.
