SYNOPSIS:

Sydney North Health Network (SNHN) invites GPs, Nurses, Pharmacists, Allied Health Professionals and other service providers working in Hornsby & Ku-Ring-Gai to attend the first Hornsby & Ku-Ring-Gai Local Coordinated Network (LCN) event. LCNs are locally driven and have been established across the region for groups of GPs, other primary care practitioners and local service providers, based around geographical regions


Hornsby & Ku-Ring-Gai Local Coordinated Networks – Building your Healthcare Neighbourhood

SYNOPSIS:

The event aims to bring together providers, managers and consumers from health and social care services across the Hornsby/Ku-Ring-Gai region with a view to improving awareness, relationships and connectivity between local services. The event will also allow attendees to work together in identifying a shared vision for what an integrated Healthcare Neighbourhood in this region could look like.
All health and social care providers and consumers with an interest in improving the connectivity of services in the Hornsby/Ku-Ring-Gai area are encouraged to attend.


Hornsby & Ku-ring-gai Local Coordinated Networks – Building your Healthcare Neighbourhood

SYNOPSIS:

This event will provide an opportunity to network with providers and consumers from the Hornsby/Ku-ring-gai area, to share resources relating to local services, and to hear about ways that the Primary Health Network, Local Health District and eHealth NSW are enabling better communication and collaboration between providers. The event will also provide an opportunity for attendees to suggest and explore ways to improve connectivity between providers, managers and consumers from health and social care services across the Hornsby/Ku-ring-gai region.


My Health Record Opt-out starts 16 July 2018 – Workshop for Pharmacists

SYNOPSIS:

This workshop will introduce the My Health Record systems to Pharmacists in preparation for the Opt-out period. It will explore the systems’ features, functionalities, benefits and uses, as they relate to pharmacy, clinical workflow and consumers. The My Health Record opt-out period is for three (3) months commencing 16 July 2018. Every Australian will then get a My Health Record, unless they choose not to.


Hornsby & Ku-Ring-Gai Local Coordinated Networks – Mental Health

This event aims to build on the work of previous Local Coordinated Network events in sharing information and fostering discussion between a broad range of providers across the Hornsby / Ku-ring-gai region. The focus of this event is mental health, bringing together providers, managers and consumers from across the Hornsby/Ku-ring-gai region with a view to improving awareness, relationships and connectivity between services and providing an opportunity to network.
All health and social care providers and consumers with an interest in improving the connectivity of mental health services in the Hornsby/Ku-ring-gai area are encouraged to attend.
