Vacancies on the ACI Chronic Care Network Executive Committee

Nominations are invited for two vacancies on the ACI Chronic Care Network Executive Committee in accordance with the accompanying Terms of Reference. The Network is particularly keen to strengthen the clinician and/or primary and community care representation on the Executive committee so anyone from general practice, primary care and the community sector are strongly encouraged to apply.


  • Active interest in the improvement of chronic care services across the state of NSW
  • Present or intending member of the Chronic Care Network
  • Prepared to participate actively in the Network activities
  • Committed to regularly attend meetings either in person or via teleconference/videoconference
  • Capacity to engender enthusiasm, and work cooperatively and effectively in a team which includes a broad range of health professionals and consumers drawn from different geographical and administrative areas


  • Attend regular bi-monthly Executive Committee meetings, and general Network meetings. Travel will be supported for regional and remote members
  • Provide advice and guidance for the Network on issues raised by Network members
  • Participate in planning and prioritisation for the Chronic Care Network
  • Strive for the provision of quality, equitable chronic care services across NSW
  • Represent the Network at forums, meetings and other relevant events
  • Willingness to contribute to Network working groups – may be required to chair specific working groups formed on the recommendation of the Executive Committee
