Sydney North Health Network is seeking to enhance access to and coordination of local primary health care services for people with chronic disease, who need better support around psychosocial, health, social and welfare needs.
Previous work with the HealthOne Project and the Local Health District, has identified that limitations and delays in psychosocial, social and welfare needs has an impact of health outcomes and the journeys of people through the health system.
We are hence inviting proposals for a Social Work Service to support GPs meet the specific needs of people with chronic disease, who need better support around psychosocial, health, social and welfare needs, in order to reduce the likelihood of admission or re-admission to hospital.
