Shape the future of the prevention and management of heart failure

Inviting expressions of interest from GPs to participate in a focus group to map out the current state of prevention and management of heart failure and then identify key areas for improvement.

Aim

To understand the current state of Heart Failure prevention and management and identify key areas of improvement required.

Description

Northern Sydney Local Health District (NSLHD) have commenced a district wide re-design of their Heart Failure Services.

They have identified that optimisation of practices across the NSLHD will result in improved outcomes for patients. This body of work will not be possible without partnerships between GPs, community health providers and NSLHD.

Sydney North Health Network (SNHN) has been engaged throughout the initial stage of the project, and input is now required from a broad group of GPs.

GP focus groups will enable the project to gain a broad understanding of the current state and the improvements required to integrate care.

Participants

We are seeking GPs to participate in a focus group to map out the current state of prevention and management of heart failure and then identify key areas for improvement. Two sessions will be run at different sites across SNPHN to enable a broad group to GPs to contribute.

Remuneration
