Shape the future of Integrated Community Palliative Care services

Inviting expressions of interest from GPs to participate in developing a plan for integrated community palliative care services by attending a co-design workshop or focus group.

Description and aim

The aim of this project is to develop a plan for integrated community-based palliative care services across the Northern Sydney Local Health District (NSLHD) region that defines the seamless and integrated journey we want our patients to have.

The output of this work will be a practical operational plan for palliative care that supports the initial implementation. The objective of the plan is to guide future service development decisions including ongoing improvement efforts, allocation of resources, funding applications, and partnerships with external services and organisations.

Participants

We are seeking expressions of interest from General Practitioners to participate in one or two co-design workshops of 3 hours each

A 1-hour focus group on a community palliative model of care and the intersection with GPs.

Remuneration

Participants will be reimbursed for their time. Food and refreshments will be provided for workshop and focus group participants.
