Sydney North Health Network (SNHN) is seeking suitably qualified and experienced organisations to apply to deliver new Mental Health Care Services in Residential Aged Care Facilities (RACFs) in the Northern Sydney region through a Request for Proposal process, with services to commence from October 2019.
Aims of Mental Health Care Services for people with a mental illness living in RACFs:
- To support people who are at risk of or have a diagnosed mental illness who are in residential care;
- To provide clinical services to help these people achieve their recovery goals;
- To improve overall mental health outcomes and equity in service availability for this cohort;
- To build RACF staff capacity.
