The Primary Care Advancement and Integration Team (PCAIT)

Sydney North Health Network’s PCAIT is a team of highly experienced primary care experts who are working with our region’s GPs, practice nurses, practice managers and practice staff to help better the patient experience.

The team is able to support your practice with accreditation, chronic disease management, CPD education, eHealth and quality improvement initiatives.

Currently PCAIT is offering a limited number of free CAT Plus licenses and incentives to support quality improvement initiatives within general practices in the Sydney north region. Each practice that takes up the offer will in addition to the installation of the CAT Plus product suite receive up to $2,000 when you participate in your first de-identified data upload and complete two quality improvement cycles.

Introducing the Community Council

The Sydney North Health Network (SNHN) Community Council is a newly established Council made up of health consumers who want to lend their voice to community health matters and needs.

The Council will work in tandem with the newly formed Sydney North Clinical Council, which is led by GPs and Health Professionals. Together these Councils will help inform the SNHN Board and Management on local health needs, priorities and gaps, helping to shape the future of local primary healthcare in our region.

The Community Council will focus on facilitating effective, person-centered primary health care to improve the overall health of our population and to reduce avoidable hospital presentations.
