HammondCare Palliative Care Support Services

Patients can access HammondCare Specialist Palliative & Supportive Care through community services located across the PHN (including Northern Beaches). Patients can receive a tailored service from a multidisciplinary team across the Northern Beaches, Upper North Shore, Lower North Shore and Ryde within their home environment or in one of our clinics. HammondCare also provides a specialist palliative care in-reach consultative service to residential aged care facilities. This service provides physical, spiritual and emotional comfort by way of skilled symptom control, good communication, and support to the patient and family. Admission into an inpatient palliative care unit is also available at Greenwich and Neringah Hospitals for complex symptom control and end of life care.

Palliative Care Home Support Program – Home based care is also available and provided by trained care workers to support the patient and family through the final days of life. The packages of 48 hours of care consist of personal care, respite, skin care, continence care, carer reassurance and support. Hours can be utilised consecutively or may be spread over days and weeks, 2nd packages can be made available. Please note: this service is accessed through your referral to HammondCare Community Palliative Care Services.

Medibank Palliative Care @ home program

HammondCare is commencing a new service in partnership with Medibank to provide Palliative Care At Home in Northern Sydney. This innovative pilot program allows HammondCare to provide palliative care to Medibank Private Members supported by a multidisciplinary team of nursing, medical, allied health and carers to patients and their families with the aim of remaining at home during end of life care. This program commenced on October 29th 2018 and the pilot will continue until June 2020. Patients will have access to a Clinical Navigator, who will provide individualised care throughout their palliative journey. The Clinical Navigator will attend to all clinical management, including complex wound care and management of syringe drivers, improving care coordination by reducing the number of services required to provide care. In addition, patients who utilise this program will have access to trained carers who can provide respite, domestic assistance and personal care in the home.
