Sydney North Health Network (SNHN) is recruiting General Practices to provide placement for graduate nurses as part of a one-year Primary Care Nurse Transition Program.
ABOUT THE PROGRAM
The Program has been specifically designed to provide knowledge, skills, support, and guidance to new graduate nurses. This enables them to become professional primary health care nurses working for the needs of the individual and community in general practice settings. The new graduate will work in general practice and become part of a multidisciplinary team in primary care.
Qualified with a Bachelor of Nursing, these nurses will be employed by you for one year (similar to GP registrar training).
They will be supported by your existing practice nurse while being coached and educated by SNHN’s Nurse Transition Program Coordinator. The Program Coordinator provides training resources, online educational material, networking opportunities and face-to-face education days during the 12-month period.

So, what can a practice nurse do?
Practice Nurses work collaboratively with GPs and other members of the Practice Team to provide safe and quality care. Practice Nurses have a variety of roles within their Scope of Practice. This includes, but is not limited to:
- Clinical skills in patient triage
- Immunisation
- Cold chain management
- Wound care
- Chronic Disease Management and Allied Health Coordination
- Preventative Health Assessments
- Patient education
- Women’s and Children’s Health checks
- Quality Improvement and Data Management
