Dying at Home
Most Australians prefer to die at home, yet few achieve it. GPs are central to changing this.
This session introduces Full Circle End of Life Matters – a comprehensive end of life team, with physician and nurse led supported model of care designed to partner with general practice. Learn how a fully coordinated, holistic approach can reduce the burden on GPs, improve communication across medical teams, and provide families with seamless end-of-life and grief and bereavement support.
With structured feedback to GPs and clear referral pathways, Full Circle strengthens the community’s ability to support dying at home while easing pressure on overstretched hospital and aged care systems.
