Outpatient services and eReferral rollout at Northern Sydney Local Health District Webinar

Are you using eReferral yet?

Since August 2022, NSLHD has processed over 10,000 electronic referrals, with more than 220 GP practices seamlessly integrating e-Referral systems into their workflows. On March 6th, an additional 98 services were added for Electronic Referral (eReferral), encompassing 90% of outpatient services across Royal North Shore, Ryde, Hornsby Ku-ring-gai, Mona Vale Hospitals, and Primary and Community Health Services.

NSLHD is currently undergoing various service development and improvement initiatives to achieve optimal patient care. The webinar covers several topics, including the progress of eReferral and other service information. This session is interactive, encouraging participants to ask questions and raise any issues. It will be beneficial for practices to utilize HealthLink in sending eReferrals to Northern Sydney Local Health District Services and receive feedback for ongoing improvements and future directions.

My Health Record for Aged Care

This webinar provides an overview of the My Health Record system and the key information it contains, including the benefits of use for aged care health providers and how residents and their representatives can use and manage their records. It will also explain how aged care services and their authorised staff can access the My Health Record system.

My Health Record for Pharmacists

This session will explain which views and documents available in My Health Record can be accessed to obtain more information about the patient’s regular medicines or medical history. You will learn how best to interpret the information available and navigate My Health Record to help you find medicines information quickly and easily when you need it.

Access to health information contained in the My Health Record can help reduce the instance of adverse medication events (including hospital re-admissions), decrease duplicate diagnostic testing and improve continuity of care for patients across the primary and secondary care sectors.

This session is suitable for both hospital and community pharmacy staff wishing to learn more about how My Health Record can help support their clinical workflow.

Electronic Prescriptions – Tips for Managing Tokens and the Active Script List

As a result, pharmacists can find themselves addressing patient queries or trouble-shooting issues when using EP as part of their clinical workflow. SNHN Residential Aged Care Virtual Service (VCS) information session

This information session for healthcare providers describes the virtual care service and platform funded by Commonwealth funding through SNHN-funded activities.

It offers telehealth equipment information for participating residential aged care homes (RACHs), secure video calls and remote observations monitoring. It is intended to be used by RACH staff, GPs and primary care providers located within the Northern Sydney PHN region.

Electronic National Residential Medication Charts

Community Pharmacists, GPs and Residential Aged Care Facility staff are encouraged to review this webinar delivered by the  Australian Digital Health Agency and Department of Health on Electronic National Residential Medication Chart (eNRMC), including its benefits and the transitional arrangements which commenced on 1 July 2022.

Under the transitional arrangement, healthcare providers can use the transitional eNRMC products for the prescribing, dispensing and supply of PBS medicines. This session will also outline eNRMC functionality that will be available in the future and provide an opportunity for questions and answers.

SafeScript NSW (Real Time Prescription Monitoring System)

SafeScript NSW provides prescribers and pharmacists with real-time information about a patient’s prescribing and dispensing history for certain high-risk medicines, known as monitored medicines which include opioids (e.g. codeine, morphine or oxycodone) and benzodiazepines (e.g. diazepam, nitrazepam or alprazolam).

This information helps to improve clinical decision making and help keep patients safe.

SafeScript NSW will be available to prescribers and pharmacists in Northern Sydney PHN region from late March 2022. Learn about the benefits of SafeScript NSW and how the system is intended to be used.

Learning Outcomes:
