Webinar: My Health Record: supporting safety, quality, and better care in Residential Aged Care Facilities (RACFs)

• How My Health Record can provide access to accurate and relevant clinical information uploaded by a range of healthcare providers (GPs, hospitals, pharmacists).
• How My Health Record in RACF’s may assist in improving quality of life, care coordination and reduce potential medication misadventures.
• How to access and appropriately integrate HealthPathways system to improve resident quality of life and patient outcomes

This session is designed for clinical staff and General Practitioners working in the Residential Aged Care setting. This session will be interactive with the opportunity for questions.

Marwa Osman – BPharm, MPS, AACPA, Digital Health Educator, Australian Digital Health Agency
Marwa is a Digital Health Educator at the Australian Digital Health Agency, who is a pharmacist by background. She is passionate about utilising digital health tools to enhance patient outcomes.

TARGET AUDIENCE:

Residential Aged Care Facility staff, GPs, GP Registrars, Nurses, and other healthcare providers who work in the Aged Care sector.
