Transforming your Practice to a Person Centred Medical Home

Sydney North Health Network (SNHN) invites local GPs, Practice Nurses, Practice Managers and Allied Health Professionals to a presentation on the key learnings from Health Care Home Practices. Drawing on experience, Dr Walid Jammal and Tracey Johnson (Practice Manager) will share their knowledge and lessons on what it takes to become a high-performing and sustainable general practice of the future.
