Advance Care Planning and Palliative Care Education Evening

Sydney North Health Network (SNHN) invites GPs, Practice Nurses and Practice Managers to attend The Advance Care Planning and Palliative Care education evening. This event will focus on better primary health care through team-based initiation of advance care planning and palliative care.

In order to get the most out of the workshop, we encourage all participants to commence the Advance ProjectTM online learning modules prior to the workshop. To access the online learning, please register for the Advance ProjectTM at www.theadvanceproject.com.au

Professor Josephine Clayton

Josephine is a Professor of Palliative Care, Medicine, Northern Clinical School and Director, Centre for Learning & Research in Palliative Care, HammondCare. She is also a Senior Staff Specialist Physician in Palliative Medicine, Greenwich Hospital.


Kelly Arthurs RN

Kelly Arthurs is the Palliative Care Clinical Nurse Consultant, HammondCare Centre for Learning & Research in Palliative Care.


GPs, Practice Nurse, Practice Managers, Practice Staff.


• Identify patients who will benefit from introduction to advance care
planning (ACP) and/or palliative/supportive care needs assessment
• Assess patients’ needs for ACP and palliative/supportive care
• Evaluate patients’ requirements for follow up and additional support
to meet ACP and/or palliative/supportive care needs
• Implement Advance Project resources in your practice

There is no cost for GPs or practice staff working within the SNHN region
