Sydney North Health Network (SNHN) invites local GPs, Practice Nurses and allied health professionals to a workshop which will focus on developing a team-based approach to patient care to improve patient outcomes, experience and increase job satisfaction for the healthcare team. High performing primary care practices are finding that creating more effective practice teams is the key to becoming a patient-centred medical home, improving patients’ health, and increasing productivity.

SPEAKERS:

Dr Kirsten Meisinger, MD, MHCDS
Dr Meisinger has been a practicing GP in the United States since 1999. Regional Medical Director and Medical Staff President – Cambridge Health Alliance; Institute for Healthcare Core Faculty; Expert panel member – Primary Care Transformation Task Force, USA and Australia; Harvard and Tufts University Faculty.

Dr Walid Jammal, MBBS FRACGP Dip. Child health M. Health Law
Dr Jammal is a GP, Practice Owner and a Clinical Lecturer at the University of Sydney and Conjoint Senior Lecturer at Western Sydney University. His practice is a lead site in the Commonwealth’s Health Care Home Trial. He has interest and expertise in care integration and the patient centred medical home.

TARGET AUDIENCE:

GPs, Practice Nurses, Practices Managers and Allied Health Professionals.

  • Explain the fundamental elements of high performing teams
  • Recognise the importance of empowerment and leadership to enabling effective team-based care
  • Describe the principles of quality and safety and how they are embedded in high performing teams
  • Explain how to use teams to underpin efficiency, best practice, and financial viability
