Navigating Frailty and Aged Care Guiding patients and their families through available services

Sydney North Health Network (SNHN) invites local GPs, Nurses, Pharmacists, Allied Health Professionals and Residential Aged Care Facility Staff to hear about:

  • A new local Specialist Geriatric Outreach service to support general practice
  • The launch of our new Frailty screening app
  • How to utilise the new HealthNavigators service to help you find local services
  • General tips on the management of frail and older people and local aged care service navigation

TARGET AUDIENCE:

GPs, Practice Nurses, Pharmacists, RACF Staff, Allied Health Professionals and their staff.

LEARNING OUTCOMES:
  •  Assess and Manage data to assist in the identification and recognition of frail and older people and people that are at risk of deterioration
  • Utilise Geriatric specialist support to appropriately manage patient intervention
  • Prevent unplanned hospitalisation of patients through ongoing co-management of patients between GPs and Geriatrician
  • Identify effective patient referral pathways and options for accessing
COST:
No cost for GPs, Practice Nurses, Pharmacists, RACF Staff, Allied Health Professionals and their staff practising within the SNHN region
