Healthy Ageing and Hospital Avoidance through Integrated Care

Join us for an evening of networking, dinner & keynote presentation by leading expert Prof Sue Kurrle

Sydney North Health Network (SNHN) invites GPs, Specialists, Practice Nurses, Pharmacists, Aged Care Providers and Allied Health Professionals to the third event in our Person-Centred, Multidisciplinary Care Symposium Series. This third session will focus on Healthy Ageing, Hospital Avoidance, Falls Prevention and how to identify Frailty.

Through a series of case studies, participants will be provided with local referral options and out of hospital services that are available. Participants will learn how to link into falls clinics or other clinics to assist with falls management for older patients, as well as Recognise the importance of a multidisciplinary model of care incorporating a person-centred approach.

Prof Kurrle is with the Faculty of Medicine, University of Sydney and Director, Staff Specialist, Geriatrician, Hornsby Ku-ring-gai Hospital and Community Services
