Preventing falls in older people: GP practice fall risk identification and tailored management

Falls are preventable – join other GPs to identify and better manage falls in older people 65 years and over in your practice, and help your older patients stay independent at home.

Sydney North Primary Health Network (SNPHN) invites GPs and Primary Health Care Nurses to attend an interactive workshop on how to identify risk factors and tailor fall management for older patients in general practice. We will also introduce a free online decision tool, fall prevention resources, and RACGP QI&CPD activity options, as well as discuss how other GP practices have implemented fall prevention in their practices.

Other health professionals are welcome to attend the workshop, however, priority will be given to those in general practice. This workshop is conducted as part of the iSOLVE fall prevention project, for more information please visit www.bit.ly/isolve.

SPEAKERS:

The workshop will be presented by Prof Lindy Clemson and the iSOLVE research team.
Prof Lindy Clemson is a Professor of Occupational Therapy and Ageing at the University of Sydney, a NHMRC CD Research Fellow and an investigator at the Centre of Excellence in Population Ageing Research. She has conducted trials in the area of falls prevention, and pioneered the “Stepping On” fall prevention program and LiFE exercise program.

TARGET AUDIENCE:

GPs and Primary Health Care Nurses

LEARNING OUTCOMES:
  • Understand the iSOLVE decision tool and resources
  • Understand the processes relevant to the iSOLVE fall prevention pathway: fall screening,
  • Fall risk assessment, tailored management and referral, and patient follow-up
  • Discuss tips on implementing fall prevention in general practice
COST:
No cost for GPs, primary health care nurses, and other health professionals working within the SNPHN region.
