After Hours Network Meeting: A Health funds role in care navigation

Sydney North Primary Health Network (SNPHN) invites local GPs, Primary Care Nurses, Pharmacists and Allied Health Professionals to an update on programs and services provided by Health funds who are currently working alongside SNPHN. They will provide information to help practices better support patients with chronic disease, encouraging self-management to reduce the risk of hospitalisation. This event will also include a panel discussion for participants to have direct interaction with the organisations representatives.

  • Participants are able to better support their patients with chronic disease by utilising health fund programs
  • Describe how health funds work collaboratively for patients, especially those at highest risk of hospitalisation and use this understanding to improve after hours access for patients to health services

No cost for GPs and health professionals practising within the SNPHN region
