Advanced Care Planning: Project Echo

Would you like access to specialist advice and knowledge on caring for a dying patient?   The Palliative Care in General Practice ECHO Network is a live, on-line learning series that includes co-management of patient cases in an interactive environment.  It will provide access to an interdisciplinary team of palliative consultants/specialists who will guide case discussion and present latest updates.

Project ECHO can support you in your work through case-based learning.  There is an opportunity for GPs to bring de-identified cases for advice and support and will also include short presentations with the next session focussing on Advance Care Planning

This session is only open to GPs.  New attendees welcome.

SPEAKERS:

Professor Katy Clark | Clinical Director of Palliative Care, NSLHD
Professor Clark is the current Clinical Director of Palliative Care for Northern Sydney Local Health District. Katy is a Palliative Care physician who has worked as a clinician, manager, teacher, and researcher in palliative care

Professor Josephine Clayton
Professor Clayton is a Professor of Palliative Care, Medicine, Northern Clinical School and Director, Centre for Learning & Research in Palliative Care, HammondCare. She is also a Senior Staff Specialist Physician in Palliative Medicine, Greenwich Hospital

Joanne Rainback | Clinical Nurse Specialist, HammondCare
Joanne works in the northern beaches community palliative care team supporting patients and families at home and in RACF. Joanne also works as part of the palliative care team in the Northern

TARGET AUDIENCE:

GPs only.

LEARNING OUTCOMES:
  • Explain the role of Advance Care Planning
  • Introduce Advance Care Planning discussions with patients, including timely communication with the family
