APAC Rebranded to Hospital in the Home

Dear GPs, Clinicians and other staff,

In response to feedback from General Practitioners, key stakeholders and a service review, APAC would like to inform you about the upcoming changes to our service commencing August 1st 2022.

These are the things you need to know:

  • The APAC service will is rebranded to Hospital in the Home (HITH) and HITH will have a direct referral line managed by HITH clinicians.
  • Referrals by General Practitioners have been streamlined and are given priority; patients accepted to HITH are clinically managed by HITH staff specialists
  • Referrers can access the HITH service between 8:00am – 10:00pm, 7 days a week.

How to refer a patient:

  • Healthlink smart eReferral is available after calling the HITH service

Common conditions that we manage:

Conditions such as cellulitis, wound infections, pneumonia, urinary tract infections, pyelonephritis, COPD, heart failure and thromboembolic disease requiring Hospital In The Home treatment.
